It means exactly what it says: the required "Skill" roll is an Attack Roll with OCV with no CSLs applied; this imposes no change on the Limitation's value. Since no other alterations of the RSR rules are noted, they apply; this includes the rules re: penalties to the roll.

I'm probably extra-dense today, but what's the DCV you're rolling to hit when appkying RSR: OCV? My best guess is zero, but I can see a case for 3 as well.

To be clear, it's not "RSR:OCV." As noted on the table on 5ER 305, it's an Attack Roll used for the RSR. It's made against the target's DCV at that particular point in time, whatever that happens to be.
