Inspired by reading Ultimate Metamorph and rethinking some characters...
Let's assume a superhuman character with three typical genre flaws and common activation conditions: OIHID or Multiform (activated by something like concentrating for a couple phases, drawing one's own blood, complex gestures, or mystic incantations, etc.), Accidental Change (something like being angry, fearful, injured, smells his own blood, experiences strong emotion or stress, etc.) and Enraged (something like being insulted or taunted, seeing friends or innocents endangered, being betrayed or cheated, etc.).
Now what about the combination of OIHID (or Multiform) and Accidental Change, since in many conditions (like the examples above) the latter would allow to activate the super-form, bypassing the usual activation requirements, in dangerous or combat situations ? OTOH, AC still creates significant problems by causing unwanted changes as well.
Also, provided that getting Enraged doesn't activate the Accidental Change on its own (an Enraged character is by definition angry, stressed, and under strong emotion), and that Enraged calls for savagely beat do the ones that touched you off by using "the most effective attacks" (i.e. those of the superhuman form) but isn't terribly conductive to thoughtful, complex actions, what should happen when one gets Enraged in normal form ?
Does the Enraged character change to superhuman form to attack (even if it requires non-instinctive actions, like concentrating, uttering phrases in mystic languages, or doing complex gestures), or charge forward in civilian form, or one should choose according to circumstances and common sense (punch the school bully in civilian form, shift to Cap. Wonderful or Bulk to beat the armed tough) ?
