Design Question: Why do all MA attacks have an even damage bonus?
defensive strike (+ 0), martial strike (+ 2), offensive strike (+ 4)
Is there a reason no strikes do +1d6 or +3d6?
Has anyone designed odd bonus MA strikes?

There are some maneuvers which add +1 DC. Legsweep is an example of this. Reversal and Root also add +15, which is an odd number too. Even is much more common though and I do not think there is any reason for it besides symetry to the eyes. :)

I've always figured that it was do to the Killing Damage/Martial Manuever thing.
When you use your maneuver with a killing attack (my martial arts style uses pole arms), you divide the DC by 2 to get the damage. By having Martial Strike + 2DC and Offensive Strike + 4 DC, then you don't have the whole half DC/round down thing.
Martial Strike with fist == Str + 2DC.
Martial Strike with naginata == Weapon damage + DC from maneuver + modified strength.
Nothing prevents you from loading up +3 DC maneuvers, just realize that if you go with a weapon style, they get dropped.

When you add DCs to manuevers that do Killing Attacks, they add at 2 DCs per Killing DC. Most maneuvers are even to facilitate this addition.
