I've read the FAQ about this question, but there are still something that I dont' understand:

If...

a) defensive action that are performed through HELD ACTION and ABORT AN ACTION alway goes first...

b) the point a) is an exception to the rules WHO GOES FIRST on 5e 235...

c) the example in 5e 235 involves a conflict between attack action and defensive action...

d) the rules in 5e 232 under SEGMENT say that each characters of equal DEX must roll 1d6 to determine who goes first...

...when and how there will be the conflict between attack and defense in the example of WHO GOES FIRST 5e 235 ???

I don't understand what it is that's confusing you. I should note, however, that if you're referring to the example at the bottom right of 5E 235, depicting a fight between Defender and Ogre, your point C is incorrect. The conflict there is between two characters who attack one another, not between a character who attacks and a character who defends.

If your question boils down to "why does the rule under "Who Goes First?" exist when all these situations seem to be covered by other rules?", the answer is: because you never know when someone will think of something that isn't covered. It never hurts to have some backup. ;)
