When I buy a HTH Killing attack it lists how much endurance it costs to use next to the power. Does that include the cost for the use of STR? My calculations would say no. By the rules you do have to pay both costs right? with running you just pay the listed cost for END, not the cost + STR right?
That second part is more of a rules question than a HD question..oh well.

You pay for the STR you add to the HKA. If you are STR 60 and have a 2d6 HKA, you can only add 30 STR to it (for a 4d6 HKA) so you only pay END for the 30 STR plus the END for the HKA. Similarly if you only add 15 STR (for a 3d6 HKA), you only pay END for the 15 STR plus the END for the HKA.
And you are right, with Running, you just pay END for the inches of Running you use -- STR is not involved so you don't pay END for it
