5e Minion Control Sheet Or Similar?

I have poked around on the website and some of my 'usual suspect' web pages, but have been unable to find a Word / Excel or similar file for the 'Minion Control Sheet' found in the Hero System Resource Kit or a similar sheet designed for 5e. Anyone know where such a thing is available?

If I may highjack your thread: How do you guys do quick-and-dirty notes of NPCs in combat? Especially including Speed and so on?
