Now couple that with the Russian idea to Nuke the wellhead.

Not only do you have poison gases and tidal waves to worry about, you would also have radioactive fallout on a scale never seen in millions of years.

The complete gulf area would be completely cleared of life. You would have extreme Global warming that would make this year look like winter. The tidal waves would possibly reach Europe and Africa. It might even set off the Madrid fault line causing a massive Earthquake in the Midwest.

This would make a great Post Apocalypse campaign.

I'm guessing much would depend on how big and dirty the nuke in question was.

... And, ummm, to take the above at face value, there were nuclear detonations (plus fall-out) millions of years ago?

(W-e-l-l. if the PCs have been doing any time travel at all, then it is certainly possible.)

There have been lots of test detonations in the last 50 or so years (all shapes and sizes, various conditions) so why would this particular nuke cause so much fall-out?

And would anybody / anything really notice (or care about) this fall-out over all the other bad stuff that would be happening more or less simultaneously?

Well, obviously, the earthquake would set it off, not that blast. That's just crazy talk.
But then the tens of millions of deaths would cause a massive surge in the background mana levels of the Earth, causing all of the Ley Lines to flare leading to interspatial portals, causing more megadeaths, and more magic surges in an uncontrollable spiral of Death, Destruction and Deadly Radiation!
