Spirit transforms

What are spirit transforms anyway? What stat do they work on; body, ego,or something else?

As noted on 5E 153, “Spirit” reflects the character’s soul, spiritual aspects, personality, and so forth. They’re most often used to represent corruption of the soul and the like; some of the demons in the HERO System Bestiary have them. Typically they work against BODY like most Transforms, but the GM might require a BOECV element if he feels that’s appropriate.
