If a character has a continuous Aid (or similar adjustment power with maximum effect), does he roll the Aid dice each phase until the maximum effect is reached, and then stay at the maximum effect for as long as the Aid remains "on"?

He rolls the dice every Phase until the maximum effect is reached, yes. This doesn’t mean the effect remains at maximum until he turns it off — the normal rules for the fading of the effect apply. For example, if the Aid is at maximum, and Post-Segment 12 rolls around, the victim loses 5 of the Aided points. The Aid will then kick in again in the “attacker’s” next Phase (probably in Segment 2 or 3). If the GM wants to handwaive that “hiccup” away, he certainly can, but that’s the strict technical interpretation of the rules.
