"Alien Physiognomy"

I've seen the above phrase used in a number of characters (Vibron, for example) as a Physical Limitation, inflicting stiff penalties on rolls to provide them medical treatment and such. My question is: why would the shape of someone's head make such a difference when trying to heal them?
