It's a Transform. You just have to define a "heal back" condition that's incredibly likely to occur (with the GM's permission, of course, but there's plenty of precedent for such things). Even in comics and gaming, "permanent" transformations aren't necessarily always as permanent as they sometimes appear... but that doesn't mean the heal back condition has to be common or likely.
In some situations, you could just model a permanent change to a character as a "radiation accident" -- i.e., an excuse to build a character with such-and-such powers, or a reason to spend Experience Points on new abilities. That might work for the T2K bit you mentioned.
