There is no formal document from the company. I think there's a thread here in the Hero System discussion forum where someone with WAY too much time layed out all the page reference changes (As in "It used to be on 402 but now is in 405").
As for changes... Steve took the entire FAQ that had been generating for years since 5E came out and integrated it into the book. They removed all the pre-5th edition art and took art from all the 5th edition sources to date and inserted it into the book instead. I believe there are a few new characters (I'm honestly not sure I ever reread sample characters), but largely the changes are "clarifications" born of the FAQ.
Someone else with a better memory will be along momentarily to point out the things I've forgotten. (That's how it works around here.)
Blue has the gist of it. Essentially you have:
Overall clearer organization and layout;
Known errata eliminated;
The bulk of the online FAQ integrated into the text, both clarifications and a few new Advantages and Adders;
More Trigger options from Digital Hero;
Some changes to Extra-Dimensional Movement (also in The Mystic World IIRC);
Several charts and other ease-of-use features from HERO System Sidekick;
Size templates from the HERO System Bestiary;
Sample characters for multiple genres, and a chargen tutorial;
Guidelines for HERO gaming in various genres (very similar to the HERO System Genre By Genre PDF);
A sample of combat (supers level).
There may be other details that I've overlooked, but I'm sure someone will fill those in.

I hope to eventually do a 5 to 5er list, just like I did for 4 to 5. However some unexpected expenses (read: having to save my cousin's financial bacon again) have pushed my purchase of 5er back again...
