In TNG and onward, it's the pips on the collar. Ship crew ranks are distinguished by the number, and whether they're solid gold or just circles. Captains wear four solid gold pips. Admirals have somewhat different insignia; second season on, it's a series of gold circles in a black rectangle, worn on both collars. And I agree with Pariah, admirals get more fru-fru.
