The left side is one of the old ones, 1-3rd edition.  In your pic the hair, cape, gloves, boots and bodysuit lines were all added in by the player. 

The right side is one of the 4th edition out of the GM Kit.  I can't find them all but here are the ones I did find.

A note, I actually liked the 1-3rd ed outlines better than the ones here.

Please do!  It's greatly appreciated!  And thank you to everyone for helping me narrow it down.

 

I found the sheet.  Attached is an initial scan, slightly cleaned up (600 DPI, rotated slightly due to the scanner, darkened some, exported to PDF using GIMP).  I haven't even tried to print it yet, so I don't know how it'll look coming out of the printer.

 

Update:  I did print it.  Not perfect but quite usable.  I've had worse copies out of a photocopy machine back in the day.  I'm heading to GameStorm over the weekend so it won't be probably until next week that I can get a better scan.

If you were to replace the Combat Maneuvers block, it would be usable up to 5th edition.  I actually had done that back in the day; in 1993ish I typed up and printed out (on my dot matrix printer) the 4th edition Combat Maneuvers and pasted it over (with a glue stick, it seems!).  Before I scanned it I peeled that off and it was as good as new.

 

Edit:  Heck, replace the stat block as well, ignore the CV and ECV blocks in the upper right, one or two other tweaks, and it would be usable in 6th edition.  :)

Awesome.  I got the 1982 files no problem.  But the other file comes up not found.

I downloaded several items and plan to touch bases with TPTB and upload them if I get the OK.

The "Free Stuff" section of the Hero Games website hosted a host of neat items which are no longer on its current incarnation -- game aids and utilities, artwork, banners and themes, adventures, free samples of articles from Digital Hero. I've downloaded many of them myself. The Internet Archive retains most of them.

I have been recovering as much as I can and uploading them again.  I have just been behind the power curve and haven't had the time lately. 

 

But locating stuff in the Archive is not my best talent....

I am also looking for the character record sheet from inside the big blue book (iirc its not at the back but earlier on) and has the picture at the bottom right but these pdf files are no longer available
