"If the roll misses solely as a result of the Behind Cover OCV penalty (i.e., it misses by less than or equal to the penalty), then the attacker may have actually hit the cover — one of the other people in the melee." Essentially - if you take a -2 OCV due to cover and miss by 1 or 2 (the range of the cover) then you accidentally hit one of the people/things that provided the cover. At least for my own Fantasy HERO campaigns I also use a 7 or less roll to possibly hit something on the other side of your target when firing into melee. Ex: The fire witch has a clear shot at the night watchman interrogating her friends. She launches a fire bolt at his back and ... misses!
