I have handy the 4th and 5.5th editions of Call of Cthulhu. The 4th ed. mainly describes the Elder Sign's use in sealing a portal. That was a major theme in Derleth's Trail of Cthulhu stories. (Which also seem to be what the board game Arkham Horror was based on.) 5.5th ed. expands on that, mentioning, that [...]the Elder Sign is worthless in personal defense if the monster or minion can evade the sign—worn around the neck, for instance, an Elder Sign might protect a few square inches of flesh but the rest of the wearer's body would be completely vulnerable. If it repels Mythos creatures maybe it just goes through whatever passes as a digestive system as it forces the "flesh" out of its way.
