Click on USER CP, go to EDIT AVATAR in the left margin, and make sure there's a mark next to "Use Custom Avatar". If there isn't, then you'll want to click on the upload button and grab it again. I think there's either an 80x80 or 100x100 size limit. If you try to make it wider/taller than that it won't upload. I think there may be a KB size limit too.
