At that point it is almost always about helping the family, while you do what you can for the patient. Doesn't matter whether it's grandma that's dying or their newborn. We've got a saying about it here. Goes like You do the most that you can, the best that you can, for as long as you can. And understand that in the end, it may not make any difference. That last is a question of borderline legality here in the US (you can't ask about family matters) but I am not interested in family issues, rather the grim side of the medicine biz.
