Well, my personal biggest mistake on an exam is a factor of 10^66.

 

I'm trying to think of what kind of question it would even be possible to get an answer that wrong on.  Something involving Planck's Constant?

No, it was a dumb mistake dropping a sign in an exponent going from one work page to the next, and it involved a factor of the solar mass, 2 * 10^33 grams.

 

I recognized very shortly there was a major mistake somewhere (to the point of writing "oh s**t" on my paper) but couldn't find it in the last couple minutes of time.

One thing I did do this year:  On one exam, the students were asked to identify the products of a reaction (lead sulfide and sodium chloride were the products).  Then, given that only one of the two products was soluble in water, I asked them to identify the product that would precipitate out of solution (lead sulfide).  A large number of students predicted that sodium chloride would be the precipitate--which would mean they didn't realize that sodium chloride was water soluble.  So on the tests of these students I wrote the words, "Where do you live?"

 

(Answer: They live on the shores of the largest saltwater lake in the western hemisphere.  Seriously, you can see the Great Salt Lake from my classroom.)
