When ever you have a hostage situation, always plan for the PCs to disregard the hostage's life. Because they do. It's not right, nor is it heroic, but it is the fault of the Player (who ultimately knows this is only a game and that there is no real life in danger).
So what does a GM do about it? Make it 'real' for the game. The hostage dies, the villains get captured and when the statements are taken from all around, word gets out (perhaps even to the media) that the hero gambled with the hostage's life and that the hostage paid the ultimate price. Inquiries, investigations, possibly lawsuits or manslaughter charges. Perhaps it gets bad enough that the hero must be suspended, or take a hiatus or sabbatical. If the hostage's death was especially careless, have the Player bring in a different PC until it all blows over (and during that time, the PC is not earning any XPs, while the rest of the team is...).
Hopefully, after all that hullabloo, the Player won't make the same mistake again.
Because it is all about consequences. In a game without consequences, Players will have their PCs do some very stupid and unheroic crap, knowing full well that they won't have to pay for it. I draw the line at docking XPs from a character, but I will reduce a PC's reward at the end of the say for poor judgement and poor roleplaying choices. Like gambing with the lives of innocent people.
