It includes cast-off pieces of non-junk (like upper stage boosters, or the launch fairing shroud panels, etc.), satellites that no longer function (the obvious ring in the image is the equatorial geosynchronous zone), and pieces of satellites that have been destroyed (the Chinese ASAT test created 2000+ trackable pieces).
