Are we too hard on Cassandra? Checking Wikipedia, the USS Fitzgerald had no lookouts posted and didn't notice the approach of the container ship until five minutes before impact, then panicked. Conversely, the USS McCain attempted to switch helm control between stations and completely botched it, completely losing control of the ship for three minutes in a crowded sea lane. Oh, I've been told to skip it and start somewhere else in the series, but each time I tried, I got the same vibe--that it was written by former "cool kids" from high school about how they thought nerds would behave in private, but it's okay because some of their best friends are smart.
